This Recommendation Form is due by November 30th. No late submissions or recommendations via other formats will be processed or considered. The Recommendation Form is completed by any alumna member of the Theta Phi Alpha Chapter/Emerging Chapter Advisory Board of the candidates choosing. If no Advisory Board exists, the Recommendation Form may be completed by the Conference Lead, Emerging Chapters Lead, or an alumna National Officer The recommender should then forward her receipt of her form to the Conference Lead for nomination.
